Job Summary
Central State Hospital seeks a full‑time Case Manager. The role primarily operates Monday through Friday, 8:00 a.m.–4:30 p.m.
Key Responsibilities- Perform case management services, including evaluation of assigned patients.
- Build rapport with patients and families.
- Provide crisis interventions, interagency conferences, agency/collateral contacts, pre‑discharge visits, and discharge planning.
- Maintain and complete service documentation such as psychosocial assessments, case notes, case reviews, court reports, letters, etc., according to prescribed standards.
- Conduct case reviews and determine current and future treatment and after‑care plans for clients completing treatment.
- Participate as an inter‑disciplinary team member on the unit regarding treatment planning, reviews, and discharge planning.
- Initiate 202A petitions; assess for guardianship and facilitate this process as needed.
- Carry out policies and procedures related to duty to warn, APS, CPS, Vine notification and other applicable legal issues.
- Write correspondence on behalf of clients and their families such as court reports and letters of recommendation.
- Conduct case presentations and consultations with other agencies about programs and services offered.
- Complete required trainings, participate in performance and quality improvement reviews, and attend workshops or seminars to promote professional growth.
- Master’s degree in social work or a related field and at least two years of experience, OR bachelor’s degree in social work or related field and enrollment in a program to pursue a master's in social work.
- Experience serving, and basic knowledge of the mental and physical characteristics of persons with serious mental illness.
- Strong communication and interpersonal skills.
- Ability to manage a case load with rapid patient turnover.
