Monday – Friday - Pay Rate: $18.00+ per hour, based on experience

- Local Travel:
Travel within the Louisville area for client visits, community outreach, provider meetings, and other business needs is required
- Experience in home care, healthcare, customer service, staffing, or coordination preferred
- Management or leadership experience preferred
- Knowledge of EVV systems such as Alora, Sandata, or similar platforms preferred
- Strong communication and relationship-building skills
- Strong organizational skills with the ability to manage multiple priorities
- Professional, calm, and empathetic approach when resolving concerns
- Comfortable working with electronic systems, documentation, and Microsoft Office applications
- Strong problem-solving skills and attention to detail
- Reliable transportation required for local travel
- Ability to work on-site Monday through Friday
- Build and maintain positive relationships with clients and families
- Assist with new-client onboarding and ongoing follow-up
- Respond to client and family questions or concerns professionally and promptly
- Help ensure clients have a positive experience with Wise Home Care
- Build supportive relationships with caregivers
- Assist caregivers with questions, updates, and office needs
- Help address caregiver and client concerns in a calm and timely manner
- Promote caregiver engagement, communication, and retention
- Support a positive and team-oriented environment
- Communicate professionally with PASSPORT case managers and other community partners
- Provide timely updates, documentation, and follow-through as needed
- Assist in maintaining strong relationships with referral sources and community partners
- Participate in provider meetings, community outreach, and local events
- Help strengthen Wise Home Care's presence throughout the Dayton market
- Maintain accurate client and caregiver records
- Assist with EVV visit corrections, compliance follow-up, and system updates
- Coordinate with scheduling, recruiting, clinical, and operational teams as needed
- Help identify and resolve client or caregiver concerns before they affect continuity of care
- Support phones, documentation, filing, and other daily office operations
- Assist with additional operational needs as assigned

Wise Home Care is seeking a relationship-driven and highly organized Home Care Coordinator to support clients, caregivers, case managers, and community partners from our Louisville, KY office. This is not a scheduling-only position. The Home Care Coordinator serves as an important connection between our clients, caregivers, office team, case managers, and community partners. The role combines relationship management, communication, operational support, problem-solving, and community engagement.
The ideal candidate enjoys working with people, can confidently manage multiple priorities, and understands the importance of timely follow-through in a fast-paced home care environment. Previous leadership or management experience is preferred for someone who can take ownership, build relationships, and help support continued growth in the Dayton market. Local travel within the Dayton area is an important part of this position and may include client visits, provider meetings, community outreach, and other business-related activities.
