This is a direct hire / permanent opportunity for a Licensed Practical Nurse (LPN) supporting home health patients in the Loveland/Fort Collins, CO area.

Location:

Loveland / Fort Collins, CO (Field-Based)

Schedule:

Full-Time | Monday–Friday | 8:00 AM — 5:00 PM

Compensation: $82,000 — $95,000 annually (based on experience)

Join a mission‑driven home health team dedicated to providing compassionate, high-quality care in the community setting. As an LPN/LVN in Home Health, you will support individualized patient care plans and deliver skilled nursing services directly in patients’ homes.

This role offers a stable weekday schedule while working in a collaborative environment focused on improving patient outcomes and supporting aging‑in‑place care models.

• Provide skilled nursing visits in accordance with physician‑directed care plans
• Deliver direct patient care in home health settings across assigned territory
• Maintain accurate and timely clinical documentation and patient records
• Monitor patient status and report changes to supervising RN or care team
• Educate patients and families on care instructions and treatment plans
• Collaborate with interdisciplinary team members to ensure continuity of care
• Travel independently to patient homes within the service area

• Active Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in the state of Colorado
• Minimum 1 year of clinical experience and 1 year of home health experience required
• Graduate of an accredited practical or vocational nursing program
• Current CPR certification required
• Valid driver’s license with reliable transportation for field visits
• Strong documentation skills and ability to work independently in a home‑based care environment

$39.00 to $45.60 per hour
