Canyon Home Care and Hospice seeks an RN to provide weekend daytime support to Hospice and Home Health patients in Northern Colorado. The role may include scheduled visits, admissions, and PRN visits to address changes in condition.
As a Registered Nurse, you work under the Clinical Manager, administer skilled nursing care per physician orders and plan of care, participate in the IDT, complete documentation, and support patients and families in their home.
