• Outpatient position with a multicultural community health center
• A team of 8 Providers, including Physicians and Advanced Practice Providers
• See 2-3 patients per hour
• 1:1 MA
• EPIC EMR
• Bilingual in Spanish, Portuguese, and Haitian Creole is a plus, but not required

• Received top-level, Level 3 recognition from the National Committee for Quality Assurance as a Patient-Centered Medical Home
• Providing patient-focused, high-quality, and coordinated care
• Newly expanded and relocated to a new state-of-the-art facility
