Position Summary:

Bright Star Care of Acton–Andover–Lowell is seeking Per Diem Physical Therapists (PT/RPT) to complete assessment-only visits for skilled home health patients. This role is ideal for clinicians looking for flexible scheduling, meaningful one‑on‑one patient care, and limited ongoing treatment responsibilities.

• Conduct initial and follow‑up assessments in accordance with physician orders and clinical guidelines
• Evaluate patient functional abilities and determine care needs
• Establish plan of care and provide appropriate clinical recommendations
• Educate patients and caregivers on safety, mobility, and home exercise strategies
• Complete timely and accurate documentation in compliance with state and regulatory standards
• Collaborate with nursing and care teams to support optimal patient outcomes

• Graduate of an accredited Physical Therapy program
• Active MA PT license.
• Experience in home health, skilled nursing, or sub‑acute rehab preferred, not required
• Strong clinical judgment, documentation accuracy, and communication skills
• Ability to work independently and travel within the local service area

Per Diem / Flexible hours ($65 to $90 per visit to client, based on experience)
