What You'll Do
  • Effectively evaluate, rehabilitate, and develop customized care plans for patients with physical functioning disorders and wounds
  • Utilize standardized assessments and evidence-based practices to support clinical interventions
  • Administer procedures including manual techniques, ambulation, therapeutic exercises, modalities, wound care, and assistive device support
  • Monitor patient progress, adjusting treatments and documenting outcomes accurately within regulatory and clinical standards
  • Communicate patient progress and collaborate with healthcare team members and families
  • Complete detailed discharge summaries and maintain thorough records in electronic documentation systems
  • Participate in meetings, trainings, and conferences as required
  • Supervise and support physical therapist assistants, rehabilitation aides, and students when applicable
  • Ensure compliance with APTA and state practice acts, adhering to the highest professional standards
What You Need
  • Valid MA Physical Therapist license
  • At least 1 year of prior LTC or rehab experience preferred
  • Strong clinical and communication skills
  • Ability to work independently and as part of a team
  • Commitment to providing compassionate, patient-centered care
