- Complete psychosocial assessments and care plans
- Join daily interdisciplinary team meetings
- Coordinate admissions and discharges
- Support and educate families on community resources
Qualifications
- 1+ year of social work experience with older adults
- Active LICSW or LCSW license
- Strong communication and teamwork skills
- Compassion for seniors and their families
