Registered Nurse II – Adult Community Clinical Services (260003TL)

Department of Mental Health (DMH)

Duties and Responsibilities
  • Provides behavioral and physical health monitoring and support; oversees, coordinates, and educates on the Medication Administration Program (MAP).
  • Completes Physical Health Assessments (PHAs) and verifies medications upon admission and during care transitions.
  • Participates in development of Individualized Assessment Plans (IAPs) and delivers trauma‑informed, person‑centered interventions focused on health and wellness.
  • Conducts routine assessments, provides medical care as needed, and ensures appropriate medical services are accessed through collaboration with care‑coordination partners.
  • Provides nursing oversight, consultation and problem‑solving to team LPNs and MAP‑certified members.
  • Serves on the ACCS Integrated Team, attends scheduled meetings, presents psychosocial history, mental status, diagnosis and treatment plans for assigned cases, and contributes to case discussions.
  • Assists in the development and implementation of IAPs in partnership with persons served and significant others.
  • Assesses ongoing needs of assigned persons, offers supportive counseling, reviews records, makes home visits and maintains regular contact.
  • Acts as advocate and liaison with primary and other health‑care providers concerning medical and medication concerns.
  • Provides rehabilitative interventions to build skills and access resources for achieving treatment goals, managing symptoms and living productive lives, incorporating SAMHSA’s 8 Dimensions of Wellness.
  • Communicates all pertinent information during daily rounds, staff meetings and notifies supervisors or designees of critical information.
  • Arranges or transports persons served needing transportation services.
  • Maintains accurate records in EMR and paper, completing all related forms to ensure compliance with DMH regulations and Rehab Option requirements.
  • Provides primary nursing care, evaluates outcomes and adjusts care in collaboration with other service providers.
  • Transcribes and implements physician orders.
  • Conducts self‑medication readiness assessments at admission and during care transitions.
  • Educates persons served on the importance of annual physical examinations, routine medical and dental consultations, and assists with coordination of these services.
  • Assists staff in meeting MAP standards and provides necessary education and support.
  • Leads or co‑leads groups focused on Health & Wellness.
  • Ensures MAP standards through education, coordination, oversight, implementation and QA reviews of the MAP process, and conducts MAP testing, recertification training and inspections of DPH registered sites.
Required Qualifications
  • Knowledge of nursing principles, terminology, and standard abbreviations.
  • Knowledge of nursing policies, procedures, methods, and techniques.
  • Knowledge of the problems in providing nursing services to special client groups such as the mentally, physically, and/or emotionally handicapped.
  • Current, valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
Preferred Qualifications
  • Certification as a Psychiatric Nurse by the American Nurses Association.
  • Knowledge of the Medication Administration Program.
  • Experience working with individuals with mental illness and co‑occurring disorders.
  • Experience in a 24/7 residential program and community setting for adults with mental illness.
  • Bilingual or multilingual ability is advantageous.
