About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by physicians, ensuring compliance with applicable laws and regulations. This role is ideal for those who are dedicated to coordinating care and maintaining continuity for patients.

• Pay: $86,767 - $130,151 annual total cash target pay

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other health care professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Deliver high-quality clinical services within the scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements, syncing timely per LHC policy.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.

• Current and unrestricted RN licensure in state of practice.

• 1+ years of Home Health experience.

• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
