Job Overview
The RN independently provides direct nursing care to patients and their families in accordance with established policies, procedures, and practices.
The Community RN Career Direct Care Pathway placement is comprised of four levels based on a RN’s minimum experience, education, and qualifications.

Career Pathway Levels- Level I – 0 – Less than 12 Months of RN Experience.
- Level II – 12 Months – Less than 3 Years of RN Experience.
- Level III – 3 Years – Less than 8 Years of RN Experience.
- Level IV – 8 Years of RN Experience.
- Massachusetts RN Licensure.
- 12 Months – Less than 3 Years of RN Experience.
- Current Basic Life Support (BLS) Certification.
- Department Specific:
Certification(s) relevant to the department and the type of patient care being provided may be required.
- Bachelor of Science in Nursing (BSN).
- Participates in and maintains awareness of performance improvement initiatives.
- Conscientiously evaluates and integrates evidence-based practice and research into clinical practice.
- Conducts and documents accurate clinical assessments.
- Develops, implements, and evaluates the patient’s individualized plan of care.
- Uses professional nursing judgment to individualize the plan of care based on assessment of the patient's needs.
- Manages own patient assignment by delegating and effectively based on patient needs and skills of team members.
- Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources, and documents according to department standards.
- Recognizes patient/family rights through support of their informed decisions and advanced directives.
- Continually evaluates and revises the care plan according to changes in the patient’s health status.
- Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.
- Guides the professional growth of department staff through precepting and mentoring.
- Participates and provides direction in department initiatives designed to improve the quality of care or clinical outcomes.
- Participates and provides direction in the development of standards of care, patient/family education, and staff education to address the needs of specific patient populations.
- Participates in departmental committees, educational activities, and research as needed.
- Participates in professional development by identifying learning needs and seeking appropriate education.
- Provides feedback to leadership regarding staff performance.
- Occasional lifting of 30-35 lbs and turning patients.
- Prolonged standing and walking.
- Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
- Exposure to infectious disease and possible carcinogenic chemicals.
- Exposure of blood-borne pathogens.
- Requires fine manual dexterity to lift and maneuver patients.
- Requires visual and aural acuity necessary to monitor patient care.
- Ability to effectively communicate with patients, families, physicians, and healthcare team.
- Knowledge of and application of the nursing process.
