Community RN – Acute Care Transition Team (Emergency Department)
The Community RN Career Direct Care Pathway placement is comprised of four levels based on a RN’s minimum experience, education, and qualifications.
• LevelI: 0 – Less than 12 months of RN experience.
• LevelII: 12–36 months of RN experience.
• LevelIII: 3–8 years of RN experience.
• LevelIV: 8+ years of RN experience.
Shift: Evenings, 32 hours per week with weekend and holiday commitment.
Location:
Lowell General Hospital, Main Campus – Emergency Room.
Join our innovative Acute Care Transition Team and help redefine the care experience for patients admitted to the hospital while awaiting an inpatient bed. As a member of this specialized team, you will provide expert inpatient nursing care, partner with multidisciplinary teams, and practice at the top of your license to improve patient flow, quality, safety, and the overall patient experience.

- Manages own patient assignments by delegating effectively based on patient needs and team member skills.
- Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources, and documents according to department standards.
- Recognizes patient/family rights through support of their informed decisions and advanced directives.
- Continually evaluates and revises the care plan according to changes in the patient’s health status.
- Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.
- Supports the development of new staff members and facilitates their transition into the unit.
- Guides the professional growth of department staff through precepting.
- Participates in professional development by identifying learning needs and seeking appropriate education.
- Participates in departmental committees as needed.
- Massachusetts RN licensure.
- Current Basic Life Support (BLS) certification.
- Department‑specific certification(s) relevant to the type of patient care being provided (may be required).
- Experience based on level: I, II, III, or IV.
- Bachelor of Science in Nursing (BSN).
- Occasional lifting of 30–35 lbs and turning patients.
- Prolonged standing and walking.
- Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
- Exposure to infectious disease and possible carcinogenic chemicals.
- Exposure to blood‑borne pathogens.
- Requires fine manual dexterity to lift and maneuver patients.
- Requires visual and aural acuity necessary to monitor patient care.
- Ability to effectively communicate with patients, families, physicians, and the healthcare team.
- Knowledge of and application of the nursing process.

Pay Range: $37.50 – $68.47 per hour.
