RN Case Manager

Responsibilities

• Manage a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions.
• Collaborate with physician, healthcare team, and the patient/family in decision making and planning of care.
• Maintain documentation of assessment findings, discharge arrangements, and actions taken according to departmental guidelines.

• Massachusetts RN Licensure.
• 3 Years of RN Experience.
• Current Basic Life Support (BLS) Certification.
• Bachelor of Science in Nursing (BSN) preferred.

Demonstrates expertise in managing patient caseloads, coordinating discharge plans, and collaborating with healthcare teams to ensure effective care transitions. Proficient in maintaining accurate documentation and adhering to departmental guidelines.
