Lowell General Hospital is seeking a dedicated RN Case Manager to coordinate discharge planning and care transitions across the continuum of care. You will work a 40-hour day shift on site at Lowell General, Main Campus, with weekend and holiday commitments.
Responsibilities include assessing patient needs, coordinating with physicians and the healthcare team, documenting findings, and facilitating referrals and insurance communication to ensure timely, cost-effective patient care.
