Community Teamwork is seeking a Lead Youth Family Foundations Housing Case Manager to join the Youth Services team at the Youth Opportunity Center in Lowell, MA. The role provides advanced case management and advocacy for youth (14-25) and their families entering housing programs.
You will supervise the Youth Family Foundations Housing team, develop networks of community resources, coordinate referrals, and ensure program entry documents are completed.
