* Required to travel between multiple assisted living facilities within Allegheny County area*

Responsibilities:

• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD-10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education

Qualifications:

• Active and unrestricted PA NP license
• Ability to complete the job's physical requirements, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform a physical assessment.
• Fully vaccinated against COVID-19
• 1 year of Long-Term Care experience preferred, but not required.
• Understanding of Geriatrics and Chronic Illness.
• Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
