Role Description

This is a full‑time, on‑site Social Worker role based in Lubbock, TX. The Social Worker will be responsible for providing support and guidance to patients, connecting them with appropriate resources, and collaborating closely with healthcare providers to develop and implement care plans. This position involves conducting assessments, crisis intervention, counseling, and assisting patients and their families in navigating medical and social services.

Maintaining accurate records and ensuring confidentiality while adhering to professional ethics is a key aspect of the role.

Qualifications
  • Proficiency in conducting assessments, developing care plans, and providing crisis intervention and counseling.
  • Knowledge of community resources and the ability to connect patients with appropriate support and services.
  • Strong interpersonal communication and active listening skills, including the ability to work compassionately with patients and families.
  • Organizational and documentation skills to manage case files, maintain accurate records, and emphasize confidentiality.
  • Master's degree in Social Work (MSW) or a related field, with current licensure in Texas (LMSW or LCSW).
  • Previous experience in a healthcare or clinical setting is preferred but not required.
  • Cultural competency and the ability to work effectively with diverse populations.
