• Contribute as a core member of an interdisciplinary primary care team, collaborating with all members of the care team toco-manage chronic conditions;
• Provide primary care consultation, evaluating patients formental health conditions, barriers to care and cognitive impairments;
• Develop and implement evidence-based treatment plans utilizing modalities like Cognitive Behavioral Therapy and Acceptance and Commitment Therapy;
• Educate patients and their support system on lifestyle modifications, behavioral health strategies and medication adherence; and
• Participate in the education of learners.
