Position Summary

As a Speech Language Pathologist (SLP), you will evaluate, develop, and implement individualized treatment plans for patients experiencing speech, language, cognitive, hearing, and swallowing disorders. You'll collaborate with physicians, caregivers, and the interdisciplinary team to help patients achieve their highest level of independence and quality of life.

Responsibilities

  • Evaluate patients and determine therapy needs
  • Develop, implement, and update individualized plans of care
  • Provide speech, language, cognitive, and swallowing therapy
  • Complete timely and accurate clinical documentation
  • Communicate effectively with physicians, patients, families, and the interdisciplinary team
  • Participate in case conferences and collaborate with the care team
  • Ensure compliance with company policies, regulatory standards, and payer requirements
  • Maintain professional competency through continuing education
  • Follow infection control and safety guidelines

Requirements

Experience

  • Minimum of two (2) years of experience as a Speech Language Pathologist in a clinical setting
  • Home health experience preferred

Education

  • Master's or Doctorate degree in Speech-Language Pathology from an accredited college or university

Licensure & Certifications

  • Current Speech Language Pathologist license in good standing
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation with current auto liability insurance

Skills

  • Strong clinical assessment and treatment skills
  • Excellent communication and interpersonal skills
  • Sound clinical judgment and decision-making abilities
  • Ability to work independently while collaborating with an interdisciplinary team
  • Ability to read, write, and communicate effectively in English

Physical & Work Environment

  • Ability to stand, walk, bend, and lift up to 50 pounds
  • Manual dexterity and hand-eye coordination required
  • Ability to meet deadlines and respond to patient needs in a fast-paced environment
  • Work primarily in patients' homes under varying environmental conditions
  • Ability to travel locally and maintain a flexible schedule
  • May be exposed to blood borne pathogens, bodily fluids, infectious diseases, and varying weather conditions

Patient Population

  • Provides care to patients of all ages in a home health setting
