Job Summary:

This position oversees the delivery of patient care and ensures high standards of nursing practice within a healthcare setting. It requires a potent combination of clinical expertise, leadership skills, and the ability to collaborate effectively with multidisciplinary teams.

Essential Functions:

  • Provide direct supervision and guidance to nursing staff, ensuring adherence to clinical protocols and standards of care.
  • Maintains a work environment conducive to achieving professional goals among employees, by providing orientation and training
  • Oversee the development and implementation of individualized patient care plans.
  • Monitor patient progress and outcomes, ensuring timely and appropriate interventions.
  • Conduct home visits as necessary to assess patient needs and provide direct patient care.
  • Identifies performance issues, discusses with Director, and assists in their resolution.
  • Supervises assigned clinical operations and ensures compliance with federal, state, and local laws and the Agency's policies, procedures, and standards.
  • Ensures all established policies, procedures, and standards of the Agency are consistently followed by employees.
  • Participate in quality improvement initiatives and lead efforts to enhance patient care quality.
  • Conduct regular audits of clinical documentation and patient records.
  • Analyzes and evaluates assigned clinical operations to ensure compliance with federal, state and local regulations.
  • Establishes appropriate standards of care and the agency performance standards (in conjunction with the Director of Health Care Services) to ensure patient/client and field employee expectations are met or exceeded.
  • Assists with the implementation of new company-sponsored systems.
  • Responsible for coding, sequencing, and diagnosis to ensure the Agency meets case weight standards.
  • Responsible for achieving required outcome levels, OASIS accuracy and compliance, and maintaining EOEs at agency-required levels.
  • Ability to effectively communicate with clinical and non-clinical staff, patients, and family/caregivers
  • Demonstrated ability to work under pressure with multiple tasks, changing priorities, and short deadlines
  • Demonstrated excellent supervisory skills
  • In-depth knowledge of OASIS documentation

  • Knowledge of healthcare regulations, standards of practice, and quality improvement processes.
  • Ability to handle multiple responsibilities and prioritize tasks effectively.

Knowledge / Skills / Abilities:

  • Able to work with people, function well under stress, use practical communication skills, set priorities, be flexible, use judgment, be creative, and accept challenges.
  • Able to demonstrate a basic understanding of computers and workplace software.
  • Able to effectively communicate with clinical and non-clinical staff, patients, and family/caregivers.
  • Organizational skills
  • Ability to supervise in accordance with Agency’s policies and applicable laws.
  • Ability to respond to common inquiries or complaints, regulatory agencies, or members of the business community.
  • ICD-11 coding capability
  • Ability to work flexible schedule and/or evening hours as needed.
  • Time management
  • Advanced written and verbal interpersonal communication
  • Basic math skills related to patient care.
  • Ability to engage in local and out-of-town travel
