Gentiva is seeking a QA Nurse to support clinical excellence across our home-based care services. In this role, you will review patient records, monitor regulatory compliance, and perform quality audits for home health and hospice programs. You'll collaborate with field clinicians and leadership to identify documentation gaps, support corrective actions, and drive continuous improvement in patient safety and outcomes. This position is ideal for a detail-oriented RN who values compassionate, home-based care and thrives in a collaborative, mission-driven environment focused on patient dignity and quality of life.

Responsibilities

  • Conduct clinical quality audits of home health and hospice patient records to ensure compliance with Gentiva policies, state, and federal regulations
  • Review care plans, documentation, and outcomes to identify gaps and opportunities for improvement
  • Provide feedback, coaching, and education to field clinicians on quality standards and best practices
  • Collaborate with nursing leadership, compliance, and interdisciplinary teams to implement quality improvement initiatives
  • Track, analyze, and report quality metrics and audit findings to support regulatory readiness and accreditation
  • Participate in root-cause analyses, corrective action planning, and follow-up reviews
  • Support orientation and ongoing training related to documentation, clinical protocols, and patient safety
  • Assist with preparation for regulatory and accreditation surveys, including documentation review
  • Use EMR and quality management systems to document audits and generate reports
  • Promote a culture of compassion, patient dignity, and clinical excellence in all quality activities
