Star Care Specialty Health System (Star Care) is seeking a full-time Licensed Masters Social Worker to join its outstanding Hospital team. The Licensed Masters Social Worker in the hospital provides essential support to patients receiving inpatient psychiatric care by assessing their psychosocial, emotional, and mental health needs and coordinating access to appropriate services and resources. They collaborate with patients, families, and the interdisciplinary treatment team to promote recovery, enhance patient well-being, and support successful discharge planning.

Responsibilities:
  • Develops, implements, and monitors quality social work services, ensuring they are available to all patients admitted to Sunrise Canyon Hospital and comply with Joint Commission, Medicare, and Psychiatric Hospital Licensure requirements.
  • Coordinates treatment planning and ensures all applicable court commitment procedures are completed in accordance with regulatory and legal requirements.
  • Facilitates a minimum of one patient group session per week and actively participates on assigned hospital committees.
  • Serves as a member of the interdisciplinary treatment team, providing professional clinical social work services to address the holistic needs of patients, coordinate care within the hospital, and promote continuity of services across the continuum of care.
  • Conducts psychosocial assessments and provides individual and family support services.
  • Collaborates with and serves as a liaison to community agencies and resources to support patient treatment and discharge needs.
  • Provides supportive counseling, completes social history assessments, develops discharge plans, coordinates post-discharge services, and delivers patient and family education to support recovery and successful transitions of care.
  • Provides individual and group therapeutic interventions as clinically indicated.
  • Maintain regular and consistent attendance for the assigned work hours.
  • Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to Star Care policies and procedures.
  • Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
  • Support all activities in alignment with Star Care’s mission, values, and operational goals.
Schedule:

This is a full-time, onsite, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications:
  • Master’s degree in Social Work (MSW) required.
  • Current Licensed Master Social Worker (LMSW) credential preferred.
  • Minimum of one year of full-time experience in a mental health, behavioral health, or psychiatric care setting.
  • Proficient in the use of standard office equipment and technology, including laptops and Microsoft Office applications (Word, Excel, Outlook, and Teams).
  • Strong organizational, communication, and interpersonal skills, with the ability to effectively collaborate with patients, families, healthcare providers, and community agencies.
