Star Care Specialty Health System (Star Care) is seeking a full-time Outpatient Substance Use Disorder Counselor to join its outstanding SUD team. The Outpatient Substance Use Disorder (SUD) Counselor plays a critical role in providing comprehensive therapeutic support to individuals receiving treatment and their family members through individual, family, and group counseling services. This position is responsible for conducting assessments to identify clinical, behavioral, and psychosocial needs;
developing and implementing individualized treatment plans; coordinating appropriate referrals to internal and external resources; and monitoring treatment progress to ensure effective outcomes. As the primary counselor for an assigned caseload, the Outpatient SUD Counselor maintains ongoing engagement with individuals throughout the treatment process, observes and evaluates progress toward treatment goals, maintains accurate documentation, and collaborates closely with multidisciplinary team members to deliver high-quality, person-centered care that promotes recovery, stability, and overall well-being.
- Provide individual and family counseling services to support recovery, treatment engagement, and overall well-being.
- Conduct comprehensive assessments to identify clinical, behavioral, and psychosocial needs and develop individualized treatment plans based on identified goals.
- Serve as the primary counselor for an assigned caseload, providing ongoing support, monitoring progress, and adjusting treatment interventions as needed.
- Coordinate referrals to internal and external resources to address medical, psychiatric, social, and community support needs.
- Maintain accurate and timely clinical documentation, including assessments, treatment plans, progress notes, and other required records.
- Collaborate with multidisciplinary team members to ensure coordinated, person-centered care and successful treatment outcomes.
- Participate in case reviews, treatment team meetings, and discharge planning activities to promote continuity of care and long-term recovery.
- Ensure all services are delivered in accordance with organizational policies, professional standards, and applicable regulatory requirements.
- Maintain regular and consistent attendance for the assigned work hours.
- Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to Star Care policies and procedures.
- Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
- Support all activities in alignment with Star Care’s mission, values, and operational goals.
This is a full-time, on-site, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

- Bachelor’s degree in Counseling, Social Work, Psychology, Human Services, or a related behavioral health field required.
