Performs advanced, specialized work to assist in the organization and implementation of various projects and programs. Work is performed under minimal supervision within established policies with latitude for independent judgment. Works on special projects as assigned. May substitute for supervisor during his/her absence.

• Establish close working relationships with patients and membersof an interprofessional team to identify non-medical drivers ofhealth and reduce health disparities.
• Promote goal setting and strategic planning for the bettermentof overall wellness.
• Accept referrals and provides case management for Children and Pregnant Women (CPW), and chronic disease management for IPCP.
• Asses healthcare needs to provide care coordination, tailoredhealth educ and identifies community resources.
• Serve as patient advocate and assists in identification and improvement of service delivery.
• Document case management activities in the electronic health record.
• Assist in the organization and implementation of various projects and programs.
• Perform work under minimal supervision within established policies with latitude for independent judgment.

• Develop and present training programs for community members,patients, and families.
• Participation in research related activities.
• Substitute for supervisor during their absence.

High school plus six years experience in related field. Additional education may substitute for experience on a year for yearbasis.

• Certified Community Health Worker (CHW) or currently enrolled and attending a CHW program that will be completed within sixmonths of employment.
• Experience with application of procedures within the CHW scopeof practice.
• Experience in dealing with behavioral health issues and/ornavigating the behavioral health system.
