A company is looking for a Comprehensive Health Assessment Nurse Practitioner.
Key Responsibilities- Conduct comprehensive health assessments and develop care plans for members using a telemedicine platform
- Evaluate risk scores, assess gaps in care, and collaborate with the care team for follow-up actions
- Participate in holiday, night, and weekend emergency coverage as needed
- At least 3 years of Nurse Practitioner experience, including 2 years in adult oncology and 1 year in telehealth
- Compact RN license and unrestricted Advanced Practice license with a willingness to obtain additional state licenses
- Experience in developing evidence-based, member-centered care plans
- Comfort with technology and virtual care platforms
- Ability to manage multiple tasks and work under pressure effectively
