Description
Hospice RN Case Manager
Provide compassionate care and meaningful support during life's most important moments.
We're looking for a compassionate and experienced Hospice RN Case Manager who is passionate about delivering high-quality end-of-life care. In this role, you'll oversee and coordinate patient care, support families through the hospice journey, and collaborate closely with the interdisciplinary team to ensure comfort, dignity, and compassionate service.
What You'll Do
- Manage and coordinate hospice patient care plans
- Perform skilled nursing assessments and ongoing patient evaluations
- Monitor patient condition and adjust care as needed
- Collaborate with physicians, caregivers, and the interdisciplinary hospice team
- Educate patients and families on hospice services, medications, and disease progression
- Provide compassionate support to patients and families during end-of-life care
- Ensure timely and accurate clinical documentation
- Maintain compliance with hospice policies, procedures, and regulatory standards
- Meaningful one-on-one patient care
- Opportunity to build strong relationships with patients and families
- Autonomy in managing patient care and scheduling
- Supportive clinical leadership and team environment
- Ability to make a true impact through compassionate hospice care
- Current, unrestricted RN license in the state of practice
- 2-3 years of clinical nursing experience preferred
- Hospice or palliative care experience strongly preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and stay organized
- Valid driver's license and reliable transportation
- Compassionate and patient-focused
- Strong leadership and teamwork skills
- Reliable and self-motivated
- Calm and confident in fast-paced or emotional situations
- Passion for providing high-quality end-of-life care
