LTSS Service Coordinator – RN Clinician
Location:
This field-based role requires the candidate to reside in the following Texas counties:
Lubbock, Hale, Wilbarger, Potter, Randall, and Wichita. Candidates must be open to traveling up to 1 hour away daily. The role involves traveling to client sites or designated locations, with occasional office attendance for meetings or training. Hybrid or virtual work is not allowed for candidates who do not reside within a reasonable commuting distance, unless an accommodation is granted as required by law.
- Perform overall management of member cases within the scope of licensure, including supervision and direction of non-RN clinicians.
- Develop, monitor, evaluate, and revise member care plans to meet needs and optimize health care across the care continuum.
- Conduct face‑to‑face clinical assessments to identify, evaluate, coordinate, and manage member needs (physical health, behavioral health, social services, long‑term services and supports).
- Identify members at high risk for complications and coordinate care with the member and health care team.
- Manage members with chronic illnesses, comorbidities, and/or disabilities to ensure cost‑effective utilization of health benefits.
- Obtain thorough member history to develop individualized care plans.
- Establish short- and long-term goals in collaboration with members, caregivers, family, natural supports, and physicians; identify members who may benefit from alternative levels of care or waiver programs.
- Develop the care plan for services and ensure member access to those services.
- Facilitate authorizations and referrals for utilization of services within benefits structure or through extra‑contractual arrangements as permitted.
- Interface with Medical Directors, Physician Advisors, and Inter‑Disciplinary Teams to develop care-management treatment plans.
- Assist in problem solving with providers, claims, or service issues.
- Direct and/or supervise work of LPN/LVN, LSW, LCSW, LMSW, and other licensed professionals; assign tasks, verify and interpret member information, conduct additional assessments, review and provide feedback on performance.
- Registered Nurse with a minimum of 3 years of experience working with individuals with chronic illnesses, comorbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role, or equivalent education and experience.
- Current, unrestricted RN license in applicable state(s).
- May require state-specified certification based on state law or contract.
- MA or MS in Health/Nursing preferred.
- Willingness to travel to work sites and other locations as necessary.
