Description

Star Care Specialty Health System (Star Care) is seeking a part-time Licensed Vocational Nurse to join its outstanding Aging & Disabilities team. The Part-Time LVN provides skilled nursing care to geriatric participants in their residences, nursing homes, and assisted living facilities while collaborating with the Health Services Director, Medical Director, and nursing team to maintain high-quality care. Responsibilities include conducting focused assessments, monitoring vital signs and blood glucose levels, administering medications and injections, managing medication refills and pill packs, providing wound care and first aid, coordinating outside medical services, maintaining accurate records and documentation, and responding to urgent health needs.

The role requires strong communication, critical thinking, clinical judgment, professionalism, flexibility, and the ability to work independently and as part of a team. The position also includes local travel, on-call and after-hours coverage, including weekends and holidays, and participation in required meetings and training.

Responsibilities:
  • Provide skilled nursing care to geriatric participants in residential settings, including nursing homes (NH) and assisted living facilities (ALF).
  • Perform focused nursing assessments, including post-hospital, fall, wound, and other participant-specific evaluations as needed.
  • Monitor and document vital signs, weight, blood glucose levels, oxygen saturation, and other health indicators.
  • Administer medications and injections and observe participant self-administration of medications when appropriate.
  • Assist with medication management, including refills, medication reconciliation, and updating medication pill packs.
  • Deliver wound care, first aid, and urgent focused assessments, and provide follow-up care for incidents or injuries.
  • Assess participant healthcare needs and coordinate referrals, hospitalizations, and outside medical services when necessary.
  • Maintain accurate inventories of prescribed and over-the-counter medications.
  • Ensure medical records and participant documentation are accurate, complete, and submitted within required time frames.
  • Complete on-call documentation and reporting requirements, including after-hours and weekend call reports.
  • Collaborate effectively with the Health Services Director, Medical Director, nursing staff, and interdisciplinary team members to ensure quality care.
  • Communicate participant status, concerns, and care needs to supervisors and appropriate team members.
  • Participate in scheduled meetings, training, and professional development activities.
  • Provide on-call, evening, weekend, holiday, and emergency coverage as assigned.
  • Strong verbal and written communication skills with the ability to interact professionally with participants, families, providers, and interdisciplinary team members.
  • Travel locally within Lubbock County to provide participant care and respond to urgent care needs.
  • Maintain regular and consistent attendance for the assigned work hours.
  • Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to Star Care policies and procedures.
  • Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
  • Support all activities in alignment with Star Care’s mission, values, and operational goals.
Schedule:

This is a part-time, on-site position with an alternating biweekly schedule that includes weekends, holidays, after-hours visits, and PRN availability. The incumbent is responsible for carrying and managing the after-hours/on-call nursing phone, responding promptly to participant needs, triage clinical concerns, and coordinating appropriate follow-up care as necessary. Must be available to provide on-call coverage and respond to urgent participant needs outside of regular business hours.
