The Clinical Manager is a full‑time, on‑site role located in Lufkin, TX. The Clinical Manager will oversee daily clinical operations for our Home Health service line, ensuring quality care for patients and their families. Responsibilities include leading and training clinical staff, coordinating home care services, implementing quality improvement initiatives, and ensuring compliance with medical standards. This role requires strong leadership, organizational ability, and a commitment to delivering compassionate care.

• Proficiency in Clinical Operations and managing day‑to‑day healthcare activities
• Experience in Training and leading professional clinical staff
• Knowledge of Home Care services and their effective coordination
• Expertise in OASIS guidelines, CMS Conditions of Participation, and quality reporting measures affecting reimbursement and agency performance
• Demonstrated skills in Quality Improvement to ensure optimal patient outcomes
• Strong problem‑solving, communication, and organizational skills
• Current and valid RN license in the state of Texas or compact state
• Bachelor's degree in Nursing or a related field (Master’s degree preferred)
• Prior leadership or management experience in a healthcare setting is an advantage
