Christus Home Care is hiring a full‑time Occupational Therapist in Lufkin, TX to work in our Home Health team.
OverviewAs the Occupational Therapist, you will provide assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. You will collaborate with the health care team and develop treatment plans based on evidence‑based therapy.
Primary Responsibilities- Provide services within the scope of practice as defined by state laws and the plan of care, using evidence‑based techniques.
- Evaluate the patient’s functional status and all body systems required for CMS documentation; consult with the physician on the therapy plan of care.
- Observe, record, and report the patient’s response to treatment and changes to the patient’s condition to the nurse supervisor and physician.
- Conduct patient assessments using diagnostic and prognostic functional ability tests.
- Current Occupational Therapy licensure in the state of practice.
- Current CPR certification.
- Valid driver’s license and vehicle insurance, or reliable public transportation.
- Ability to function in any home situation, regardless of client characteristics such as age, race, creed, color, sex, disability, or financial condition.
- Demonstrated ability to manage multiple tasks simultaneously.
- Demonstrated ability to work independently.
- Strong communication, writing, and organizational skills.
- Annual Total Cash Target Pay: $75,998 – $113,997
- Hourly Pay Range: $36.54 – $54.81
- Per Visit Point Pay Range: $43.85 – $65.77
