Sevita - - Responsibilities:
Deliver direct support per individual service/program plans; assist with daily activities (personal care, housekeeping, recreational activities, transportation, community integration, shopping);
Record progress and activity; maintain confidentiality and review logs;
Support health care needs including medication administration as assigned; accompany to medical appointments when needed;
Support transportation and community involvement; ensure safety and adherence to policies;
Participate in training and team meetings; follow safety procedures and report hazards
