Role Description

This is a full-time, on-site Nurse Practitioner role based in Lufkin, TX. The Nurse Practitioner will be responsible for providing primary face to face hospice visits and palliative support for patients, by conducting assessments, developing care plans, and collaborating with interdisciplinary teams. Key tasks also include monitoring patient progress, educating patients and families about care plans and conditions, and ensuring high-quality end-of-life care.

The position requires adherence to clinical protocols and a compassionate, patient-centered approach.

Qualifications
  • Advanced clinical skills, including patient assessment, diagnosis, and treatment planning
  • Strong communication and interpersonal skills for patient and family education and collaboration with care teams
  • Knowledge of hospice and palliative care practices, end-of-life care, and pain management
  • Experience in maintaining accurate and timely medical records and following healthcare compliance standards
  • Ability to work compassionately with patients in sensitive situations
  • Master’s degree in Nursing and current licensure as a Nurse Practitioner (NP) in the state of Texas
  • Minimum of two years of clinical practice; hospice or palliative care experience is preferred
  • CPR certification and any relevant certifications in palliative or hospice care are beneficial
