Position Highlights

Accountable for supporting patient care navigation in the clinic setting. The RN's primary responsibility is to manage care for patients of the primary care practice to promote effective education, self‑management support, and timely health care delivery. This includes developing and monitoring care management processes, supporting primary clinical teams, identifying high‑acuity patient populations, and ensuring care coordination. The RN will work with clinic leadership, medical director, and Care Coordination/Medical Home teams to support quality measures in the medical home model.

What

you will do
  • Serve as a resource for all clinical teams on care management of all practice patients, educating patients on self‑management tasks.
  • Manage patient care across the health care continuum to achieve optimal outcomes in a safe and cost‑effective manner.
  • Collaborate with physicians, providers, and practice staff to identify appropriate patients for care management and provide leadership for patient care teams.
  • Work with social services and nursing to plan for and provide appropriate follow‑up care for patients.
  • Communicate care plans to patients, families, medical providers, hospital staff, community support, and other agencies.
  • Provide follow‑up contact with patients to ensure compliance with medication, labs, specialist visits, PCP visits, dietician, CDE, etc.
  • Review monthly status reports, dashboard results, and other information related to clinical care delivery.
  • Communicate with physician groups regarding statistical and financial impact of care delivery.
  • Serve as a resource for clinic care team education on changing payor requirements.
  • Collaborate with payer Case Managers for additional services when appropriate.
  • Participate in regular team meetings and peer review activities.
  • Anticipate the needs of the patient population, ensuring necessary documentation and pre‑visit planning is completed before patient visits.
  • Promote patient self‑management and empower patients/families to achieve maximum levels of wellness and independence.
  • Assess barriers when patients have not met treatment goals or followed treatment plans.
  • Determine and complete appropriate referrals.
  • Oversee the development, procurement, and adoption of patient self‑management educational resources used by primary clinical teams.

Required Education, License/Certification, or Work Experience
  • Registered Nurse (RN) – Board of Nursing
  • Active license in the state of practice upon hire
Preferred Education, License/Certification, or Work Experience
  • 1–3 years of clinic nursing experience
  • 4–6 years of case management and/or homecare experience
