Clinical Care Leader (CCL)
OverviewThe Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized nursing care in the home setting.
ResponsibilitiesCollaborate with inter‑professional colleagues to plan, implement, and evaluate care. Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations. Utilize technology to enhance quality resident care, collaborate with team members, delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment, and serve in leadership roles on department/organizational task forces and committees.
Demonstrate competency within the full scope of nursing expertise and practice. Communicate effectively to collaborate, negotiate, and resolve conflicts with peers, colleagues, residents, and families. Apply strong clinical reasoning and critical thinking to analyze data and respond to changing situations.
- Bachelor’s degree in Nursing strongly preferred.
- Graduate from an accredited nursing program (e.g., CCNE, ACEN, NLN CNEA).
- Minimum of two years of clinical experience preferred.
- Current, unencumbered RN license with the state board where practice occurs; multistate licensure acceptable.
- Required:
Basic Life Support (BLS). Resident Assessment Instrument (RAI) experience preferred. - Valid driver’s license may be required depending on location.
Salary Range: $38.50 - $55.00 per hour, plus a $20,000 sign‑on bonus, shift differential, holiday pay, paid time off, and other benefits.
