Position: PACE Nurse Practitioner: Elder Care, Mon–Fri, 5 Patients/Day

The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long‑term care, or assisted living.

Centra’s PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our care model. Responsibilities
  • Monday – Friday 8:00 am – 5:00 pm – no weekends or holidays.
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers.
  • 5 patients per day – small volume.
  • Call is once every 7 weeks, 5 pm – 8 am; you are not expected to go out, calls are triaged before reaching you.

Qualifications
  • Graduate of an accredited nursing program and Master of Science Degree in Nursing (MSN).
  • Minimum of five (5+) years of progressive experience in area of specialization.
  • Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS certifications. Holds a current, active license as a registered nurse in Virginia or holds a current multi‑state license privilege as a registered nurse. Nationally recognized certification in field of specialization.
  • Experienced providers in palliative, hospice, primary care, or long‑term care are encouraged to apply.
