Job Description- Administer skilled nursing care for clients of all ages in their place of residence.
- Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Provide education to patients and their families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to home health aides to ensure service quality.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Perform on-call responsibilities and provide on-call services as needed.
- On-call required if requested; on-call needs are subject to change based on staff and branch needs.
- Current state license as a Registered Nurse.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- CHPN certification preferred.
- Proof of current CPR certification.
- Benefits are available to full-time employees after 90 days of employment.
- A 401(k) with company match is available after 1 year of service.
