Job Description

We are seeking an exceptional and compassionate Nurse Practitioner or Physician Assistant to join our dedicated Palliative Care team. This is a meaningful opportunity to make a lasting difference in the lives of patients and families during some of their most vulnerable moments.

In this role, you will provide holistic and supportive care for patients with advanced illnesses — primarily in oncology, neurology (stroke, dementia, etc.), pulmonary (COPD, respiratory failure, etc.), and gastrointestinal diseases. You’ll be part of a collaborative interdisciplinary team focused on improving quality of life and managing symptoms.

Responsibilities

Responsibilities include:

  • Provide holistic palliative care for patients with advanced illnesses in collaboration with an interdisciplinary team.
  • Assess and manage symptoms to improve quality of life across oncology, neurology, pulmonary, and GI conditions.
  • Support patients and families with care planning, advance care planning, and end-of-life care decisions.
  • Coordinate with physicians, nurses, social workers, and other care teams to ensure comprehensive symptom management and transition planning.
  • Participate in a structured orientation and ongoing professional development as part of a collaborative health system.

Required Qualifications

  • Experience with disease processes and symptom management pertinent to palliative care.
  • Experience with end-of-life care and advanced care planning.

Qualifications

Required

Education:

Graduate of an accredited Nursing Program, and Master’s of Science Degree in Nursing

Preferred Experience:

Minimum of two (2+) years of progressive experience in area of specialization

Required Certifications and Licensures:
Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization
