Overview
We are seeking a compassionate and experienced Registered Nurse – Integrated Care Manager (ICM) for a temporary assignment (approximately 3 months, with potential for extension) located in Lynchburg, VA
. The role requires the ability to conduct face‑to‑face clinical assessments with members residing in the Lynchburg area of VA. Candidates must reside close to this area.
First (Days)
Responsibilities- Conduct face‑to‑face clinical assessments with members.
- Perform telephonic assessments.
- Identify high‑risk conditions and coordinate care plans.
- Assist with authorizations, referrals, and resource connections.
- Ensure compliance with regulatory and organizational standards.
Required: Bachelor of Science in Nursing (BSN).
Certification and LicensureRequired: RN license in the state of Virginia (or multistate compact license).
Experience- Experience in case management, care coordination, or utilization/care review.
- Experience conducting clinical assessments (telephonic and/or face‑to‑face).
- Experience supporting members with chronic conditions, co‑morbidities, disabilities, or complex care needs.
- Strong communication, clinical judgment, and problem‑solving skills.
- Ability to engage collaboratively with providers, interdisciplinary teams, and members.
- Experience with health‑plan or managed‑care settings preferred.
