Role Overview
Provide comprehensive in-home health assessments to adult and geriatric members, reviewing medical history, medication adherence, social environment, and health risks to improve care quality and identify undiagnosed conditions.
Key Responsibilities- Conduct assessments, including medical history, diagnosis, treatment, and health education
- Administer point-of-care and lab tests as needed
- Collaborate with physicians, case managers, social workers, caregivers, and other medical personnel
- Coordinate follow-up and provide patient education in members’ homes or facilities
- Master’s Degree or equivalent with NP licensure and prescriptive authority
- Board certification in relevant adult or family care specialties
- Current BLS, ACLS, or CPR certification
- Valid driver’s license and automobile insurance
- Strong communication, computer, and documentation skills
