Care for individuals recovering from critical illness who need ongoing respiratory support, complex wound care, intravenous therapies, cardiac monitoring, dialysis, and comprehensive rehabilitation services before transitioning to a lower level of care. - Additional Info:
We utilize the LEAF system, a wearable patient monitoring technology that helps staff track and document patient repositioning. Our unit provides dialysis treatments at the bedside using TABLO dialysis machines. This innovative technology allows patients to receive dialysis without leaving their room, improving comfort and continuity of care.
- Beds: 26-bed Long Term Care Hospital
- Patient Population:

The Wound nurse serves as the clinical resource and consultant for the acute care of patients with selected disorders of the integumentary systems The wound nurse should be prepared to provide the full scope based on certification and professional education nursing practice including education.
Responsibilities- Identify patients at risk for pressure ulcer/pressure injury development and provide appropriate preventative care using established protocols
- Identify patients at risk for skin tears and or incontinence associated dermatitis and initiate appropriate preventative care.
- Identify and address causative and contributing factors to skin breakdown.
- Performs the following procedures: swab cultures and negative pressure wound therapy.
- Creates and maintains a personal learning plan.
- Incorporates health education as an integral part of nursing practice in identifying and meeting patient/staff needs.
- Maintain and submit required activity and statistical reports to unit manager.
- Obtain certification as a WOCN within an allotted time frame that is based on the shortest time attainable.
- Other duties that may be assigned from time to time.
- Participates in learning opportunities to enhance professional growth.
- Participates in Quality Improvement activities.
- Provide appropriate written information to patients and staff including postoperative and discharge instructions referrals and supply needs.
- Provide consultation and direct assistance to nursing staff on issues related to patients with altered skin integrity secondary to pressure necrosis drainage to the level of scope from certificates in wound management.
- Provides coaching/counseling for staff to improve professional/technical skills.
- Provides learning opportunities for health team members.
- Serves as a clinical consultant to patients and health care professionals in specialty.
- Utilize and maintain current physical assessment skills and interview techniques to determine patient needs.
- Utilize the nursing process when directing or delivering care to patients identified.
- Utilizes research-based data to improve patient outcomes.
