Licensed Practical Nurse; LPN; LPN

LPN – Home Health

Schedule:

8-hour shift

Assignment Details:
Contract length: 8 weeks, Guaranteed hours: 40 per week

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
• Proof of current Hepatitis consent/declination and TB vaccination record
• Ability to use electronic medical charting and time-keeping software (Homecare Homebase)

Responsibilities:

• Assist in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs under the direction of a Registered Nurse (RN).
• Participate in planning, coordinating, and implementing care in conjunction with the RN.
• Provide effective communication to patients/families and other healthcare professionals.
• Assist patients/families in learning appropriate self-care techniques and assess responses to care.
• Utilize infection control measures to protect staff and patients.
• Assist with the admission, discharge, and transfer of patients.
