Location:

Lynchburg, VA

Salary Range: $23.25-$26.50 per hour
Stipend for after hours emergency rotation

Summary

Join the Humankind Foster Care team and become an integral part of a team dedicated to the success of every child under our care. As a Case Manager, you will work in a supportive and collaborative environment where connection and teamwork thrive.

Responsibilities
  • Complete and maintain thorough documentation, including Monthly Notes and Reports, 14-Day Plans, 30-Day Plans, 45-Day Plans, Social Histories, Quarterly Reports, and Annual Reports.
  • Assess the needs of foster children and implement tangible objectives, strategies, and services to address all identified needs.
  • Conduct home and field visits multiple times a month for each child.
  • Attend or coordinate meetings on behalf of foster children and families, including FAPT, Court, FPM, Treatment Teams, and VEMAT meetings.
  • Supervise visitations between foster children and their birth families, provide transportation for identified services, and make service referrals to various agencies.
  • Participate in ongoing trainings required by LCPA standards, Human Kind, and supervisory staff.
  • Participate in all audits, including internal and external conduct by LCPA Licensing Inspector.
  • Use Placement/Matching protocol when assistance is needed for placement by the Program Manager.
  • Provide ongoing support and training to foster families.

Requirements for Consideration
  • A baccalaureate degree in any field plus two years of experience in providing casework services to children and families.
  • OR a baccalaureate degree in social work or a related field (e.g., sociology, psychology, special education, counseling) and one year of experience in providing casework services to children and families.
  • OR a doctorate or master’s degree in social work from an accredited college or university (or a field related to social work) and one year of experience in providing casework services to children and families.
  • Valid driver’s license and acceptable driving record.
  • One year of case management experience, preferably with children and families.
  • Complete, pass, and maintain all required background checks and drug tests.
  • Ability to respond to on‑call emergencies within an approved timeframe as determined by Program Manager.
