Position Overview
The Order Tracking & Face to Face Coordinator is responsible for managing, processing, and tracking medical orders to ensure accuracy, completeness, and regulatory compliance. This role supports clinical teams, physicians, and administrative staff by maintaining timely workflows, updating records, and providing support for key documentation functions such as Face-to-Face (F2F) processing.

- Order Tracking
- Track and process outgoing and incoming medical orders according to established procedures, ensuring all required information is complete.
- Follow up with clinicians and managers when orders are returned due to incorrect physician information.
- Utilize a document management system to follow up on outstanding orders weekly and document tracking notes for all follow-up actions.
- Monitor the EMR fax system, review all incoming orders for completeness, upload/check in appropriate orders, and follow up on any incomplete documentation.
- Update physician information in shared files, patient profiles, and outstanding orders as requested by clinical staff.
- Travel to physician offices to pick up orders when requested by the manager.
- Face-to-Face (F2F) Coordinator
- Review all F2F documents to ensure they meet Medicare criteria supporting home care eligibility. Process F2F documents in the client record according to procedure.
- Track outstanding F2F documentation using a document management system and perform regular follow-up.
- Notify the admitting clinician when certifications or F2F documents are not created in a timely manner.
- Coordinate with the clinical team to ensure MD appointments are scheduled when progress notes are unavailable and no appointment has been scheduled.
- Run F2F events reports and reconcile discrepancies between the document management system and upcoming patient admissions.
- Additional Backup Responsibilities
- Provide backup for VNA Order Tracking.
- Participate in reception coverage rotation.
