Licensed Social Worker – North Shore, MA area including Lynn
Full-Time | Monday–Friday | Day Shift (until 4 PM)
We are seeking Licensed Social Workers (LICSW/LCSW) to support geriatric patients through a mix of in facility and patient home/facility visits.
Key Responsibilities- Participate in IDT assessments, meetings, and care planning,
- Complete social work assessments and maintain timely, accurate documentation.
- Provide psychosocial assessments, supportive counseling, and collaborate with health providers.
- Coordinate hospital, rehab, and nursing home admissions/discharges
- Support transitions to long‑term care and collaborate with Medicaid Specialists, SNFs, and caregivers.
- Facilitate family meetings and connect families with community resources.
- Assist with guardianship updates, health care proxy education, and insurance‑related documentation.
- Review care plans with participants, guardians, and/or activated HCPs.
- Complete authorizations for home care and other approved services.
- Conduct home and SNF visits as needed; support palliative and end‑of‑life planning.
- Communicate updates regarding disenrollment, LTC conversion, site transfers, and demographic changes.
- Report allegations of abuse and complete required documentation.
- Active Massachusetts LCSW or LICSW
- Experience with geriatric populations
- Strong assessment, communication, and documentation skills
- MA Driver's License
