Element Care Inc. is seeking an Advanced Practice Provider (NP/PA) to join the PACE interdisciplinary team. The APP will collaborate with Medical Doctors to deliver comprehensive primary care for geriatric participants across homes, ALFs, and centers.
The role is full-time, Monday through Friday, 8 a.m. to 4 p.m., with frequent local travel and compliance with MA licensure and DEA requirements. Prior geriatric experience and strong teamwork are essential.
