Position Overview
As a Continuous Care RN, you will provide intensive, shift‑based, 1:1 clinical care to manage uncontrolled symptoms such as pain, dyspnea, agitation, or respiratory distress. You’ll collaborate closely with the interdisciplinary team to stabilize symptoms and ensure comfort, dignity, and safety.

- Provide direct, continuous bedside nursing care during periods of medical crisis
- Assess, monitor, and document patient condition and response to interventions
- Administer medications (including rapid titration as appropriate) and perform skilled nursing procedures
- Educate and support families during moments of uncertainty or distress
- Communicate effectively with the hospice medical director, RN case manager, and interdisciplinary team
- Ensure compliance with hospice regulations and All Care clinical standards
- Also provide per diem admission and revisit assistance when needed
- Valid Massachusetts Registered Nurse (RN) license
- Minimum 1 year of nursing experience (hospice, critical care, med‑surg, ICU or ED strongly preferred)
- Strong clinical assessment and symptom‑management skills
- Ability to work independently in the field
- Compassionate communication and crisis‑support skills
- Reliable transportation; ability to travel within the service area
- Ability to be available for same day for Crisis intervention
