We are looking for a RN Case Manager to join our team in a hybrid remote/field‑based role. This role is 70% remote and 30% travel.

The RN Case Manager provides comprehensive case management for assigned members with complex needs, ensuring cost‑effective quality care. This professional delivers quality patient care via the development, implementation, and evaluation of individual patient care plans. The role adheres to established professional standards of care, and the Element Care policies and procedures. It is a full‑time position, Monday‑Friday 8‑4, with no nights, weekends, or holidays.

Responsibilities
  • Develop a plan of care that aligns with physicians’ orders, based on assessment of the patient’s needs, condition, environment, and consultations with other health team members; update the plan of care regularly and document this care following Element Care procedures.
  • Ensure the delivery of competent, quality patient care in the home.
  • Maintain ongoing responsibility for the assigned caseload.
  • Oversee the implementation and evaluation of individual patient plans of care.
  • Maintain communication with Commonwealth Care Alliance (CCA), Element Care staff, and the provider community as appropriate.
  • Provide patient education opportunities.
  • Work with both members and their family members.
  • Actively participate in team knowledge‑sharing activities.
  • Perform other duties as assigned.
Qualifications
  • Registered Nurse, currently licensed in the Commonwealth of Massachusetts.
  • Bachelor of Science degree in Nursing preferred.
  • Minimum of three (3) years of experience as a licensed, registered nurse; preferably five (5) years, including home‑care clinical experience.
  • Proficient in the use of Microsoft Office tools.
  • Experience working with case‑management data systems.
  • Ability to travel locally on a frequent basis.
  • COVID‑vaccinated preferred.
  • Spanish speaking preferred.
