RN Home Health Visits

Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to provide skilled home health visits to patients in their homes. This position begins as part-time and is ideal for an RN who enjoys independent work, flexible scheduling, and one‑on‑one patient care.
Driving to patient homes is required.

• Provide skilled nursing care in patients’ homes per physician orders and care plans

• Perform comprehensive assessments, vital signs, and skilled nursing interventions

• Administer medications, wound care, injections, and treatments

• Educate patients and caregivers on medications, disease processes, and safety

• Monitor patient progress and report changes in condition

• Complete accurate, timely documentation for each visit

• Communicate with physicians, case managers, and interdisciplinary team members

• Follow all home health regulations and agency policies

• Active Massachusetts Registered Nurse (RN) license in good standing

• Minimum 1 year of RN experience (home health experience preferred, not required)

• Strong assessment, documentation, and communication skills

• Ability to work independently and manage time efficiently

• Valid driver’s license, reliable transportation, and willingness to travel

• Comfortable using EMR and mobile documentation systems

• Per-diem, Part-time to start

Pay: $40.00 - $45.00 per visit

SOC - $150

Experience:

• Nursing: 1 year (Required)

• Home health: 1 year (Preferred)

License/Certification:

• RN License (Required)

• CPR Certification (Required)
