Registered Nurse - New Graduates

Join a dedicated interdisciplinary team providing community-based nursing care for frail elders. This full-time nursing role involves assessment, care planning, and care coordination to support participants' health and social needs, promoting their ability to live safely in the community.

• Conducting initial and ongoing assessments of participants.
• Developing and updating personalized care plans in collaboration with the team and family.
• Monitoring health status, identifying urgent needs, and ensuring tests and diagnostics are completed.
• Providing direct nursing care with professionalism and respect.
• Participating in care plan implementation and evaluation to meet quality standards.

• Current RN license in the specified state.
• At least two years of geriatric nursing experience.
• Proficiency in Microsoft Office tools.
• Ability to travel locally using a personal vehicle.
• Strong problem recognition and solution skills.
• Preferred:
  Bilingual in Spanish and vaccinated against COVID-19.
