Centerboard Inc. seeks a dynamic Case Manager to provide direct support, advocacy, and service coordination for youth and families in our residential programs. You will work closely with Clinicians and the Clinical Director to ensure high-quality resident care from intake through transition.
You will manage admissions, maintain documentation, and coordinate with DCF and partner providers, while promoting safety, dignity, and community living with a family-centered approach.
